THE CHARNWOOD PRACTICE APPOINTMENT SYSTEM

Further to the success of the new appointment system we introduced last December we are making a few additional changes so that we can continue to provide a high quality of care to all of our patients.

Please note, the following changes will take place with effect from Monday 3rd October 2016

  • If you require a same day appointment you must call the surgery before 12 midday.

 

  • Currently all same day appointments will be telephone triaged first by a member of the clinical team . This will continue for all GP appointments and with effect from 3rd October will include appointments in the Minor Illness Clinic to ensure they are all appointments are offered based on clinical need rather than on a first come first serve basis.

Please note, same day appointments are intended for emergencies and urgent care, if your medical issue is of a non-urgent or on-going issue please accept any advanced booking offered to you by the reception team. Alternatively advanced appointments can also be booked online. Please ask a member of the reception team if you have not yet registered for online services.

  • Any requests for home visits must be made before 12 midday.

 

  • If you do not require an appointment but instead require medical advice or need to speak to the GP you must call before 12 midday. Providing you do call before this time you will receive a call back from the on-call GP on the same day.

Please note, you are not able to choose which GP you speak to nor be able to request a specific time for a call back.

With effect from Monday 3rd October any requests for same day appointments or requests for non-urgent call backs made after 12 midday will be asked to either call back the next day or signposted to one of the other services available to support GP practices.

ADDITIONAL GP SERVICES AVAILABLE TO YOU

Recognising the difficulties people face juggling the demands of modern life the local CCG has commissioned a number of services for patients in the city to offer more flexibility to enable them to access additional GP service when there own GP has reached full capacity for appointments available in a given day.

1.The Healthcare Hubs

There are 4 hubs across the city ran by GP’s available to all patients registered at a City GP. The locations are:

Weekday Opening Hours - 6.30pm to 10.00pm

Saturdays, Sundays and Bank holidays – 12 midday to 8.00pm

Appointments in the Hubs are pre-bookable - Simply telephone 0116 3660560 during opening hours to book an appointment. Alternatively you can attend your chosen Hub as a walk in patient provided this is after 6.30pm.

  1. Digital GP

Digital GP provides telephone consultations directly with a GP, from 8.00am to 8.00pm, 7 days a week.

Patients should call 0116 366 0560 and select option 2 on the telephone system for phone advice between 8-8pm seven days a week. The GP will call the patient within a maximum of one hour.

The GP on the telephone will have full access to the patient’s record and so will be able to make a well informed decision about what is best for the patient. The Charnwood Practice will be notified of the outcome of the consultation in the same way as they are with appointments attended at the healthcare hubs.

Please note, the Digital GP Service is for medical advice Only. Prescriptons and sick notes cannot be requested through this service.

Practice Policies

Confidentiality & Medical Records

Locked blue folderThe practice complies with data protection and access to medical records legislation. Identifiable information about you will be shared with others in the following circumstances:

  • To provide further medical treatment for you e.g. from district nurses and hospital services.
  • To help you get other services e.g. from the social work department. This requires your consent.
  • When we have a duty to others e.g. in child protection cases anonymised patient information will also be used at local and national level to help the Health Board and Government plan services e.g. for diabetic care.

If you do not wish anonymous information about you to be used in such a way, please let us know.

Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.

Freedom of Information

Information about the General Practioners and the practice required for disclosure under this act can be made available to the public. All requests for such information should be made to the practice manager.

Access to Records

In accordance with the Data Protection Act 1998 and Access to Health Records Act, patients may request to see their medical records. Such requests should be made through the practice manager and may be subject to an administration charge. No information will be released without the patient consent unless we are legally obliged to do so.

Complaints

Customer service formWe make every effort to give the best service possible to everyone who attends our practice.

However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible.

Violence Policy

The NHS operate a zero tolerance policy with regard to violence and abuse and the practice has the right to remove violent patients from the list with immediate effect in order to safeguard practice staff, patients and other persons. Violence in this context includes actual or threatened physical violence or verbal abuse which leads to fear for a person’s safety. In this situation we will notify the patient in writing of their removal from the list and record in the patient’s medical records the fact of the removal and the circumstances leading to it.
